Pool Manager Forms

Prospective Natural Gas Providers:

  • To be placed on the Approved List of Providers, you must complete a Pool Manager Agreement Form and meet the requirements listed on this form. Once accepted, your information will be placed on our website and made available to consumers
  • The Pool Manager shall have the right to terminate service to a customer on the first day of any month by providing a Pool Manager Termination of Service Form no fewer than 30 business days prior to the desired date of termination

The customer must complete a Request for Enrollment Kit and submit all necessary documents in order to enroll in the Provider Choice Program

The customer will have the right to change their elected Daily Delivery Quantity (DDQ) on the first day of any month, but must provide a DDQ Change Request Form no fewer than 10 business days prior in order to do so

The customer will have the right to change their elected Pool Manager on the first day of any month, but must provide a Pool Manager Change Request Form no fewer than 10 business days prior in order to do so

The customer can return to FPU’s regulated natural gas service on the first day of any month and must provide a Request to Return to Regulated Sales Service Form no fewer than 10 business days prior in order to do so
